In Singapore, Human Organ Transplant Act (HOTA) covers all Singapore Citizens and Permanent Residents of 21 years old and above. HOTA allows for kidneys, liver, heart and cornea to be removed and used for transplant purpose, when a person dies. You can choose to opt-out from HOTA by filling up the form and send it to the National Organ Transplant Unit.
CPF Nomination provides an option to specify who will receive your CPF Monies according to your wishes. If you do not make a CPF nomination, CPF will distribute your savings to your family members under the Interstate Succession Act. You can nominate who to receive your CPF monies using the CPF nomination form that can be found in CPF website.
A Will is a legal document that states your instructions and wishes on your assets distribution after your death. If you do not have a will, the state will distribute your assets according to the Interstate Succession Act.
Advanced Care Planning (ACP)
Advance Care Planning is the process of making your future health and personal care decision in advance through conversations and discussions. The medical team will act accordingly to your decision when treatment is needed. Your love one will have a peace of mind from not having to help you decide on what you might or might not want when you lose the ability to speak for yourself.
Advanced Medical Directive (AMD)
An Advance Medical Directive (AMD) is a legal document that you sign in advance to inform your doctor that you do not want the use of any life-sustaining treatment to prolong your life, in the event that you are terminally ill, unconscious or incapable of making decision. The AMD overrules even family members objection to your refusal of life-sustaining treatments.
Lasting Power of Attorney (LPA)
The Lasting Power of Attorney is a legal document that allows a person who is at least 21 years old to appoint one or more persons to make decision for you if you lose your mental capacity in the future. Choose your preferred proxy decision maker that can be trusted to make decisions at your best interest.
Pre-plan your funeral according to your choice and wishes. You can approach any funeral company that you trust to make the arrangement. Pre-planning your funeral allows your family to respect your decisions, and leave no financial burden on them.
